We are looking for a dedicated, compassionate and energetic nurse for a client in Brookfield who has cerebral palsy. Accompany him to his last year of High School and in the home on evenings and weekends. Duties include medication and tube feeding administration via G-tube. Assisting with transfers and ADLs. Collaborating and communicating with his parents while following the plan of care. Start this week and meet this fun loving, adventurous family! Shift: Tuesday and Wednesday from 7:00 am to 3:30 pm at school Weekends from 3:00 pm - 10:30 pm ** Other evening opportunities are also available Responsibilities

Nurse would accompany client to school 2 days a week
Provide thoughtful care to client & patients based on personalized Plan of Care
Monitor and track changes in the client/patient’s conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
Proactively updates client/patients’ family on plan of care, health changes, and concerns

Requirements

Current unrestricted LPN or RN license in state for which care is being provided
Certified CPR and negative TB skin test or chest x-ray
Minimum one (1) year of documented experience required in a home health agency, primary care clinic or health facility, preferred.
Preferably with experience with G-Tube and bolus feeding
Preferably with valid driver's license, car insurance and reliable automobile
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. *Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
